How to manage microclimate in the plus sized patient?

According to a 2016 report by the WHO, globally there are 650 million people that are obese accounting for approx. 13% of the world’s adult population¹.
Caring for a plus size patient is a substantial and increasingly necessary challenge for today’s healthcare providers. This is especially so in the hospital environment, where the increasing population of larger patients places considerable pressure on available resources.
Without proper equipment, both caregivers and patients can be at risk of serious injury. Plus size patients commonly suffer increased skin fragility as a result of fluid retention and poor circulation – making them more vulnerable to pressure injury and skin tears2. To help mitigate this risk, expert body guidelines such as the European Pressure Ulcer Advisory Panel (EPUAP) and National Pressure Ulcer Advisory Panel (NPUAP) recommend selecting a support surface with enhanced microclimate features, pressure redistribution and shear reduction3.
What is microclimate and why is it important in pressure injury prevention and management in the bariatric patient?
The term microclimate is used to describe the local tissue temperature and moisture at the body and support surface interface. Excessive moisture on the surface of the skin reduces patient resilience to withstand pressure, shear, and friction – some of the key risk factors that lead to the development of pressure injury4.

Obese patients tend to sweat more profusely in an attempt to cool the body and regulate temperature increasing the risk of excess moisture at the skin/surface interface. Furthermore, increased temperature caused by heat accumulation where the body is in contact with the support surface typically leads to sweat at the back and buttocks, particularly vulnerable areas for pressure ulcer formation.
